Intaking form
We would like to collect some data from you to enable to us to contact you efficiently and alert your GP if we ever felt you could be a risk to yourself or someone else. Data is held for 5 years in line with guidance from our insurer. All data is stored in line with GCPR policies and will not be shared with 3rd parties.
